Supervised training and therapy at home

The theme supervised training and therapy at home deals with remotely supervised preventive and therapeutic training of patients in their daily life context at the time and place of their choice: addressing matching, mentoring and monitoring (MMM). The training itself may involve very specific training of e.g. hand and cognitive functions, or more general training of physical condition and mental training. Matching involves who is doing what, where, at what time and at what costs (e.g. professionalism, competences, procedures, etc.). Mentoring involves classic and modern coaching tools (interactive patient-held electronic health record, e-coaching, and serious gaming modalities), personalized and context aware feed-forward and feedback provision with help with, amongst other things, patient profiling models and serious gaming modalities to enhance motivation and training effects.

Medium term
We expect the following results in this theme within 5 years: the concept of MMM will be ready and available for chronic care in general, and cost effectiveness will be proven for treatment and chronic preventive disease management (e.g. COPD, chronic pain, arthritis, diabetes and chronic heart failure).

Long term
For the long term we expect the further development of the MMM concept into general and specific modules per disease. In this theme we expect the following results within 5 years:

  • the concept of MMM ready and available for chronic care
  • cost effectiveness will be proven for treatment and chronic
